The Child MyLeightons Contact Lens Care Plan
Please read the following information and click 'NEXT' to confirm that you agree:
- The monthly fee for your My Leightons Care Plan is £12.00 and will be taken out on your nominated payment date.
- Please note that the care plan does not cover your standard eye examination which will normally be covered by the NHS
- There is a minimum term for membership of 12 months. If you cancel during this time you will be required to pay the remaining term or fees for services used (whichever is smaller)
- You can discuss your plan at any time by emailing patientsupport@leightons.co.uk or calling 0800 40 20 20
- By continuing to sign up to the the MyLeightons Care Plan your information will continue to be shared with ADARO Optics Ltd who are a data processor to fulfil your ongoing orders and to provide payment services via direct debit.
- We are committed to the highest level of data privacy standards and patient confidentiality. Your data will be collected and processed in accordance with the Privacy Policies of both Leightons Opticians & Hearing Care and ADARO Optics Ltd. Both of these policies can be accessed below, and links will be emailed to you as part of your welcome email.
- If you join an NHS MyLeightons Care plan and are not entitled to an NHS contriduction towards your eye examination you will be transferred onto our private plan (£16/month) when you attend for your appointment.
